Make The Switch

Step 1 - Contact our customer service department

You can reach our Customer Service Department by calling 616-913-8075. Our Customer Service representatives are available Monday through Friday 8am –5 pm to answer your questions and register you with one of our board certified physicians.   Please have your insurance card available when you call.


Step 2 - Transfer your medical records

In order to obtain a copy of your medical records we need you to complete an Authorization for release of medical information form.  Completing this form and returning it to your previous physician will allow your physician to send your records to your new physician.  This form will be included in the packet of new patient information we will mail to you when you call to register with one of our physicians.  You also have the option to download this form and print it from your computer.  Please request your records at least two weeks prior to your first visit with us. Step 3 - Notify your insurance company

Some insurance companies require notification when you change your primary care physician.   You can notify your insurance company by calling the phone number located on the back of your insurance card.  Please contact our Customer Service Department at 616-913-8075 If you would like help understanding your insurance requirements.
